Sample Participants Mailing List--Text Format
ASPCS Volume XXX

To prevent duplication of efforts and to expedite shipping and e-book 
access, we would appreciate your mailing list in the Excel form 
available at www.aspbooks.org, if possible.  If you do not have 
Excel or Open Office, we can accept your mailing list in a text format 
following the layout given below; however, we cannot accept PDF and 
postscript mailing lists.

Institutional Mail Box, Code, etc.
Institution/Corporation/Company
Department
Street Address
City/Town
State/Province
Postal/Zip Code
Country
Phone number (including country and area code)
E-mail address
Shipping Method (use exact phrase or number each time)

The shipping method options are as follows:

1 USPS Media Mail 
     (2-3 weeks, 1 book)
2 USPS Priority Mail Flat Rate 
     (2-4 days, 1-4 books to single address)
3 International First Class
      (avg. 2-4 weeks, 1 book)
4 International Priority Mail Flat Rate 
     (6-10 days, 1-4 books to single address)

As of May 14, 2007, other forms of shipping were discontinued.
If any editors or participants should receive more than one volume, 
please place those individuals at the beginning of the list, 
preceded by the number of copies to be shipped to that individual, 
as in the following example:

10 copies

etc.

Please include as much information as possible. If there is no 
information for a particular field, please simply type "(none)". 
This can be submitted in a plain text or rich text format 
(a file with the extension .txt or .rtf), which we can then 
import into Excel. Examples are noted below:
